CMS plans to revise the medical coding system that physicians and hospitals use to bill health insurers, a move that many health care providers and health insurers support but that "could initially cause headaches for consumers and their doctors," the Wall Street Journal reports.

The revised system, called ICD-10, will increase the number of codes used to define various diagnoses and medical procedures by almost 10 times to 155,000 in an effort to account for the large number of medical advances that occur annually. The revised system will increase the number of codes used to define diagnoses to 68,000 from 13,000 and the number used to define medical procedures to 87,000 from 3,000.

According to CMS, the current system, which the U.S. adopted in the 1960s based on a framework developed by the World Health Organization, lacks space to expand, an issue that required the agency to group some new medical procedures with unrelated diagnoses. Karen Trudel, deputy director of the Office of E-Health Standards and Services at CMS, said, "The lack of space is a symptom of the fact that medical procedures are changing so quickly."

CMS, which proposed the revised system in August, hopes to have the system implemented fully within the next three years. CMS estimates that adoption of the revised system will cost the health care industry $1.64 billion over 15 years.

My favourite place? My very special backyard haven.

This backyard was built and grown with love. Our family has lived in this house since 1964 so the different transformations that have happened in this yard have been dramatic over the years, from the massive cedars in the very beginning, to the roses and bonsi juniper that are still here today.

In the late '60s, my father invented an all-season patio cover on wheels.

This fabulous outdoor fixture (with electricity) protected our family from the elements for years, and still does. This cover is on wheels so that you can move it around the backyard where you want it. It has stood the test of time.

There is also an attachment that hooks onto it to cover the stairs.

It has had different decor through the years -- everything from lanterns to balloons -- but my favourite is the way it is now. It has become our Shisha tent. We entertain and dine under it all the time and it is always a conversation piece.

Case in point: We reviewed the Panasonic DMR-EH75V about a year and half ago when it had a list price of $500. If you search for the DMR-EH75H on Amazon, you'll see three resellers (all with reputable ratings) selling them for insane prices:

Is it just a glitch with Amazon? You might think so, but that's pretty clearly not the case. Jumping over to eBay, we found some recent auctions for the same product.

Sure, that's less than half of the Amazon prices, but they're still very high. How often does the price of electronics increase after being used for a few years? What's going on here?

The most important section of your business plan is its executive summary. That’s right. It’s the first thing that people read, and it should crystallize everything that’s compelling and essential about your business and how you will succeed with it. And you need to do that all on one page. Gulp.

If you’re looking for investors, they won’t read past the first page if it doesn’t grab them. That doesn’t just mean great writing, but also great content that demonstrates that you know what you’re talking about.

Who will be running your business is clearly the most important part of your business.  But for the business plan, it’s the executive summary that has to be good or they’ll never get around to reading about your amazing management team or anything else in your plan.

Now, please don’t conclude from this that a compelling executive summary is all you need. That compelling executive summary only works if it’s well supported in the body of the plan. Grandiose assertions don’t win over investors, and a business strategy built around such assertions will fail also.

So you need a powerful plan that’s topped off with the best executive summary you can write. And rewrite and rewrite and rewrite until it’s just right.

Mandalay Bay is one of our favorite hotels. Why? Well, we love that the lobby (impossibly high ceilings, calm, gleaming with marble, and housing a large aquarium), and the other public areas really do make this seem more like an actual resort hotel than just a Vegas version of one. You don't have to walk through the casino to get to any of these public areas or the guest-room elevators, the pool area is spiffy, and the entire complex is marginally less confusing and certainly less overwhelming than some of the neighboring behemoths.

We wouldn't say it really evokes colonial Southeast Asia -- oh, maybe around the edges, if you squint, thanks to the odd bit of foliage or Balinese carving. This may well keep out the gawkers, who are looking for bigger visual thrills, but we find a place whose theme doesn't bop you over the head refreshing.

The freshly redone spacious rooms are among the most desirable on the Strip. There is no tropical influence; they've gone with geometrics, like everyone else, though theirs are very handsome indeed. The bathrooms are the crowning glory: downright large with impressive, slightly sunken tubs, glassed-in showers, double sinks, and separate water closets, plus lots of fab amenities. Rooms on higher floors have some of the best Strip views in town, but usually cost an additional fee.

The paper shows that upward pressure on prices remains limited in most categories of products and services, while wage pressures remain under control.

Note that meeting management skills are often helpful in designing an effective presentation. Also note that the following guidelines are intended for general presentations, not for training sessions where your presentation is to help learners to gain specific knowledge, skills or attitudes in order to improve their performance on a task or job.

Basic Guidelines For Designing Your Presentation

1. List and prioritize the top three goals that you want to accomplish with your audience. It's not enough just to talk at them. You may think you know what you want to accomplish in your presentation, but if you're not clear with yourself and others, it is very easy - too easy - for your audience to completely miss the point of your presentation. For example, your goals may be for them to appreciate the accomplishments of your organization, learn how to use your services, etc. Again, the goals should be in terms of what you want to accomplish with your audience.
2. Be really clear about who your audience is and about why is it important for them to be in the meeting. Members of your audience will want to know right away why they were the ones chosen to be in your presentation. Be sure that your presentation makes this clear to them right away. This will help you clarify your invitation list and design your invitation to them.
3. List the major points of information that you want to convey to your audience. When you're done making that list, then ask yourself, "If everyone in the audience understands all of those points, then will I have achieved the goal that I set for this meeting?"
4. Be clear about the tone that you want to set for your presentation, for example, hopefulness, celebration, warning, teamwork, etc. Consciously identifying the tone to yourself can help you cultivate that mood to your audience.

Shipping container homes just keep on getting cooler. Developers Katie Nichols and John Walker along with architect Christopher Robertson wanted to create affordable and sustainable homes for the emerging hipster crowd – modern, colorful and creative. This single-story home, located on the outskirts of downtown Houston in a “transitional neighborhood,” is made from 4 shipping containers sourced from nearby ports. The house is constructed using some fairly advanced building techniques that make it an extremely sturdy and well insulated structure, not to mention incredibly cool. 

If you’ve taken to shipping container architecture, you probably already noticed the interesting design of the home, use of space, and range of ceiling heights. Building with shipping container is a bit like playing with legos – you’ve got certain constraints, but they can be arranged in any number of ways. There are 4 containers, three of which are 40-foot high cubes (9′6″ tall) and a fourth is a 20-foot standard (8′6″). Two of the 40-foot containers make up the living, dining, and bedrooms, while the 20-foot container is slightly elevated and serves as a galley kitchen. Many of the interior walls were taken out or re-purposed to yield a surprisingly open and airy feeling home. The last 40-foot container is set across an open breezeway and serves as the guest cottage, totaling for a modest 3-bedroom home of 1,858 square feet.

The use of shipping containers means that the structure of the home is essentially prefabricated when it arrives on site. Each of the containers cost $2,000 to $5,500. The containers were placed on the site within one day, and within one month the home was enclosed and ready for interior work. Supported on 34 small piers elevated off the ground, the containers are less susceptible to settling and seasonal movement.

Making it Work
Essentially, satellite TV dishes are a special type of parabolic reflector antenna that is designed to focus on a larger broadcasting satellite that is situated in geostationary orbit. Upon receiving the signals transmitted from the broadcasting satellite, the satellite dish funnels this signal into a receiver that processes this signal and converts it into the sound and picture that is broadcast on your television.

Satellite TV dishes are a particular type of microwave antenna that is available in various sizes and designs. A standard satellite TV dish is comprised of a parabolic (bowl-shaped) surface and a central feed horn. The receiving satellite TV dish uses its curved dish, which receives signals from a broadcasting satellite, to reflect the signal inward into the feed horn.

This is similar to a concave mirror that focuses light onto a particular point. The feed horn holds a low noise blockdown converter (LNB), which amplifies the signal transmitted from a broadcasting satellite that is bouncing off the dish and filters out the noise. As a result of the LNB, an amplified and filtered signal is sent to the satellite receiver that is inside the viewer’s home. The viewer is then able to see the sound and visual content that is being broadcast from the broadcasting satellites.
